Patient Information


At your first appointment, Dr. Herd will complete an in-depth medical and dental health history; an oral cancer screening; and a thorough examination for bone loss, loose teeth, bite, TMJ and other signs of periodontal disease. We will be as gentle as possible during our examination. We will take time to get to know what you want as far as your treatment and will discuss possible options with you.

Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and X-rays from your referring dentist.
  • Bone loss can be seen on X-rays. We will need current X-rays in order to see disease not otherwise visible, or to “see” what is going on below the gum tissue. If your referring dentist has taken X-rays, you may request that they be forwarded to us. If not, we may need to take X-rays, especially since they are required for insurance claims.
  • A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website.
  • If you have dental insurance, please bring any forms or insurance cards with you to the appointment.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to this appointment (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes or hypertension).


PerioIndy is aware that your time is valuable and strives to answer your calls Monday – Friday from 8-6 PM. We currently treat patients Tuesday (8-6 PM), Wednesday (12-7 PM), and Friday (8-5 PM). Dr. Herd teaches at the Indiana University School of Dentistry on Mondays and Thursdays, so we assure you she is not out playing golf those days! We will schedule your appointment as promptly as possible. Please allow approximately 1 hour for your initial visit. If you have pain or an emergency, every attempt will be made to see you that day.


We are in network with several insurance networks. Each one is vastly different in terms of coverage and benefits. Some companies offer many different dental and medical plans. These companies can change benefits, co-pays and deductibles many times throughout the year. We do our best to provide you with accurate coverage estimates based on information available to us. At times, it is almost impossible to accurately estimate a patient’s insurance co-payment. Many insurance companies will not give out fees until after the treatment is completed. Dealing with these companies can be difficult and time-consuming. As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance is current.


Our mission is to deliver the finest and most comprehensive periodontal services available today. We are concerned about your dental care and want to assure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy.

Payment Options:

In general, payment is due in full at the time of service. We will be more than happy to bill your insurance carrier for all procedures but will request your co-pay at the time of your appointment. For your convenience, we accept cash, checks, money orders and credit card payments (MasterCard, Visa and Discover).

Payment Plans:

We do not arrange payment plans through the office. For your convenience, payments may be arranged with CareCredit, which offers no-interest financing. Visit for more information.
